CHF Patients At Home

Been working hospice for about 6 months now and I've noticed CHF patients are some of the most difficult to care for. Two person assists to transfer patient are problematic for most families as well as the hospice team. Lift chairs are nice but expensive and donated chairs are rare. Hoyer lifts are a possibility, but most elder spouses aren't likely to be able to work them. The non-emergent fire dept can assist but not on a regular basis. What are some out of the box options enabling families to care for and keep loved ones longer at the home?
