Does anyone here have a sample of the PRC form for operating room and delivery room cases? PLS? if you're not allowed it to post as an attachment here.

We have a Personal Messaging (PM) system here where users can just click on the recipient's name and choose to send a PM or e-mail. Thank you.

i think you should ask for a copy frm ur school coz as far as i know your prc forms should have logo ( like a letterhead) of ur sch..

Try to ask your level coordinator regarding your PRC form. Info about your school is different from that of other schools. Your coordinator should have provided you with the format indicating the Area manager with signatures and places you had your duties....
