Question Starting Independent Contractor RN business

Am interested in starting my own business as an independent contractor RN providing nursing services in the home. I need help getting started. Have questions re:getting started in general, how to bill (insurance, Medicaid, & Medicare), marketing, incorporating, and fees schedule for services provided.
Would appreciate any advice/help.

I am starting a independant RN contracting and medical liasion/advocate on myown.
How is yours going?
What did you find out about roadblocks etc?
