Ok-Got the Job! Now what???

Today I was offered the job at the agency I wanted to be in so badly...
The pay is just a little less than what I make now but with 40hrs totaled it is actually more.
I have to call and talk to my current manager to give my 2 weeks notice and I am so scared about making such a huge change, but it is really what I want to do. Ilove what this job offers. No more night shift.
The thing that gets me worried is that summer is coming and I wouldn't have those great 4 days off with my children. They are older, but still being gone M-F seems weird.
Thank you everyone here for your good advice. Now how to tell current manager??

Easy. Just write a formal letter of resignation, making certain that you mention positive things about the job you're leaving, sign it, and take it in with you. Ask to see the manager for a moment, hand it to her, and say you've decided to try out home health. She won't kill you or serve you to the others for dinner. Good luck.

First of all, congratulations ! Good for you! Your manager will probably be surprised at first if she doesn't suspect, but will then probably offer you her best wishes. It's by trying different things that you'll be able to learn about them, as well as learn where you want to be 20 years from now. I was worried about that when I first started trying different specialties, but over the years the comments became more and more positive about "all the experience I have". It's really a win-win situation for all involved.

Good Luck, I left the PM thing those 12's turned into 14's of course. Just tell them you need to change due to family needs/ consistent scheduling, that youv'e enjoyed working there etc, etc, never burn your bridges. Since I have kids and a self-employed husband (ie no medical), I opted for weekend baylor/ home hospice 2 8a-6p on sat/sun full benies, and I felt it would solve all the child care/scheduling issues, plus give me m-f home all summer w/them. You need to do what feels right, each person's situation is unique
