Looking for sage advice - PLEASE advise :)

HI

I have taken on the position of helping a small mom & pop Personal Care agency get going.

They have been doing just private pay personal care with just a couple of clients but have applied for and received Medicaid certification in our state.

My problem of the moment is this - the state provides a care plan form for the personal care aides that is difficult to use and has no provision or latitude for personalization of care. The state web site says that an agency can create their own format as long as it meets state guidelines. I could create such a form from scratch but would rather not reinvent the wheel. I've used good aide care plans in the past but alas have kept no samples.

I have done some searches on the web but have not been successful.

Does anyone know of a source or resource that could be helpful in this endeavor?
