You might want to go under Nursing Specialty (see the Specialty Tab above) and post this under the Home Health Folder.

I work in home health...but I don't know what you mean by a "Travel Chart." Could you be a little more specific?

a chart to take to the clients home. for the follow up visit, like the 485. I put together the clinical chart for the office. I guess it would be considered the field chart
