'early intervention'

Hi all,

I’m collecting information for a proposal for a study on whether regular monitoring and assessment of seniors would result in lower medical costs and reduced hospitalizations.

My hypothesis is that regular assessment of at risk populations would enable early interventions thereby catching and reversing illness before they require acute care.

My problem is whenever I search ‘early interventions’ I receive information on pre-school issues, ADHA, ABS, etc.

Does anyone have any ideas as to where I might find this kind of information, or what a more appropriate terminology might be?
