If I tell her that I'm very sorry but I'm still thinking about it and she calls other people I might be stuck without a job if I don't get the OR offer. But if I work out a start date, fill out the paperwork and DO get the other offer I would feel really crappy quitting.

Call and and pester them about the OR internship. Tell the LTAC that they will have to wait. Do what you love or you will hate what you get stuck doing. Besides, I have a feeling you have the OR postition anyway...CONGRATULATIONS!!
