I have a new order to increase a medication to 4 times p/day instead of 3 times p/day. Do I need to D/C the original order and then add a new order in the MAR? Please help I'm having a brain "fart!"

Yes, if you don't it may cause a medication error. You don't want another nurse to come behind you and give more or less than what the current order states.

In my LTC facility ... in the MAR ... If a med is D/C’d we put “X” in the blocks for the remaining times for that day followed by a large “D/C”. If a med is changed we put “X” in the blocks for the remaining times for that day followed by “See N/O”.

A new order is a new order and should be treated like any other new order. So yes, you discontinue the old order, mark MAR accordingly, and write new order on MAR.

Of course you need to write a new order...dc the old order on the MAR and add the new one.

yes hun you discontinue the previous phy order and write new phy order using discontinue not d/c (its no longer valid with BON) also note this on your MAR and report it to oncoming shift...if its a med change we also notify pharmacy and RP (responsible party)...
