Our SS dept. does MDS/ RAP (we use the progress note)/ and POC for mood & psychosocial only. Coordinators do delirium, cognitive & behavior; we helped write the policy for which discipline did which sections.

In my facility, SS does cognitive, behavior, mood, and psychosocial. Our SS has been with the company 15 years and is a God-send. Of course, if I read something that I disagree with, I have the ultimate decision.
