Forms, forms , forms. Any advice on where to locate sample forms from 24 hour report to daily documentation to nurse aide documentation?

Most facilities won't share their forms with you. You might find something you can use here, but you need an Acrobat Reader to open most of the files:

Yes! Briggs! When I made my post I was trying to think of their name and could only remember that it began with a "B". They have all kinds of nursing forms. This company sells the heavy plastic chartbacks that facilities keep charts in as well. We used to buy myelar hole reinforcement strips from them for our MAR and TAR repairs because they do a superior job of reinforcing the notebook holes at the top of those pages when they start to break through than anything else we had ever seen on the market.
