I had a patient come in under medicare for two days and left, I can do the 5 day with the second day as the ARD but what about the OBRA ? Do I code it 5 day for medicare then do a d/c prior to initial assessment for OBRA or would they think as I had done a 5 day that could be used as an initial assessment. ?? I have been doing MDS for 2 1/2 years and believe it or not this is the first medicare to leave prior to 5 day for me. I thought I had done it all !!!!!

All that needs to be done is a discharge. Tax

You do the 5 day for PPS, ( have to if you are going to bill for this stay.) no RAPS etc, then do the discharge prior to inital assessment for OBRA. On the 5 day assess, in section AA8a code-0 for none of the above and in section AA8b code-1 for 5 day assess. I ran into this all the time. Hope this helps.

Make sure you send the 5 day before the discharge if they have the same ARD. Otherwise the 5 day will be refused. had this happen , too. yuk. what a mess.

Thanks everyone for your input I called the Help desk and they told me exactly the same 5 day then d/c prior to admission; what a great site this is just found it.
