Pre-op screening criteria for egd/colonoscopy in amb surg center

We want to establish specific pre-op screening criteria for certain diagnoses and problems when plannning egd/colonoscopies in our ambulatory surg center. Does anyone have specific guidelines or policies (and be willing to share) for any of the following?
Obesity, sleep apnea, CAD, Diabetes, Pacemaker, pregnancy, breatfeeding, fever/respiratory infeciton, CHF, S/P MI, COPD.

We are pretty stringent. ASA status 2 or less. No oxygen, no defibs (will take pacemakers). We do go to "light" status 3's for some things. For example S/P MI is technically a three, but if it was 20 years ago and no further problems. The key is a good H&P and an itelligent look at the risks involved. No pregnancy either. Others are OK as long as they are well controlled.
