yes, go to the chart. Look at the patients history and the present medical diagnosis. What caused the stroke, What do they need to do to correct the stroke, what will be included in the rehab, assess their mental status and do a diagnosis for that also. Are there any other medical issues involved?

I am moving your post to the Student Assistance forum where it is more appropriate. But I do need to add in that we do not do your homework for you here, we will be glad to offer advice, or critique what you have find things written, but again, we will not do it for you.

There are many links that can be found on that forum as well as assistance in how do look up articles, or do research, but you will need to do the work on your own.

Do you have a care plan book or access to a list of nursing diagnoses? Either of those would be a good place to start with formulating these diagnoses based off of your patient's symptoms gathered from your assessment. As far as prioritization goes, think about the Maslow's hierarchy of needs and organize the diagnoses according to that.

We don't know any data on your patient and how she is reacting to her stroke. Nursing diagnoses are based on assessment data of the patient and their symptoms or reactions to situations, treatment and the like. What kind of stroke was it? Does she have any modifiable risk factors that you can help her work on so as to prevent further incidences of a stroke? Does she have any residual symptoms, such as dysphagia or vision issues? Is she at risk for falls? Does she have any pre-existing issues that are causing her issues as well, such as hypertension?

I will help students with care plans but I will not give you any nursing diagnoses until you have shown me that you have done a thorough assessment of this patient first and need assistance yourself on how to make a determination from that information what the nursing diagnoses (nursing problems) are. You won't learn how to diagnose if I just give you nursing diagnoses. Nor will you learn anything about prioritizing (http://en.wikipedia.org/wiki/Maslow's_hierarchy_of_needs).

The patient's age and medical diagnosis don't carry as much importance in the care planning process as your nursing assessment of their response to this situation. What you need to do is:
  • a physical assessment of the patient
  • assess the patient's ability and any assistance they need to accomplish their ADLs (activities of daily living) with the disease
  • collect data from the medical record (information in the doctor's history and physical, information in the doctor's progress notes, test result information, notes by ancillary healthcare providers such as physical therapists and dietitians
  • know the pathophysiology, signs/symptoms, usual tests ordered, and medical treatment for the medical disease or condition that the patient has. This includes knowing about any medical procedures that have been performed on the patient, their expected consequences during the healing phase, and potential complications. If this information is not known, then you need to research and find it.
From all of that information, you assemble everything that applies to this patient that is abnormal. That abnormal information becomes the basis for the nursing diagnoses that you will choose. You will need a nursing diagnosis reference book to choose nursing diagnoses correctly.
