HELP....please..

can any body help me do my care plan, i lost my nanda book, the problem is about dm type 2...tnx for your replies...+ respect

Please be aware that we will assist you, but there is no one that is going to do your homework for you. You are going to need to do the required reading for it and even getting the basic ideas down, and then if you have specific questions about it, we will be only to happy to assist.

You are also going to need to get another book, especially if you are just starting your program. No other way to do things.

All care plans begin by doing a thorough assessment of the patient. This includes:
  1. a physical assessment of the patient (if this is a real patient; if this is a hypothetical patient see #4).   2. assessment of the patient's ability and any assistance they need to accomplish their ADLs (activities of daily living) with the disease (if this is a real patient; if this is a hypothetical patient see #4)
  3. data collected from the medical record (information in the doctor's history and physical, information in the doctor's progress notes, test result information, notes by ancillary healthcare providers such as physical therapists and dietitians (if this is a real patient; if this is a hypothetical patient see #4)
  4. knowing the pathophysiology, signs/symptoms, usual tests ordered, and medical treatment for the medical disease or condition that the patient has. This includes knowing about any medical procedures that have been performed on the patient, their expected consequences during the healing phase, and potential complications. If this information is not known, then you need to research and find it.
From your assessment you make a list of all the signs and symptoms as well as the behavior responses you see in the patient. This list becomes the foundation of everything else you do in making the remainder of the care plan
  • using the NANDA book to help you determine the patient's nursing diagnoses
  • developing goals (objectives)
  • writing nursing interventions

Not to heap a bunch of criticism your way, but seriously..... Your patient has an extremely common disease, that could have thousands of individual interventions, goals and expected outcomes. Not to mention their own specific laboratory findings and Hx's and medications..... DO YOUR OWN WORK!!

sorry to rant, but it is frustrating to see this type of request.
