Smile Health Assessment Resources, Techniques, and Forms

Ok I need pointers I am in my first semester nursing and we have evals on health assessment head to toe UGH! I am freaking I was very sick the morning we had lecture on this and am in need of some kind of checklist or video to show me more of what to do anyone out there have any hints for me pleaseeeeeeeeee Thank you all!

This is a GREAT form my clinical instructor made for to us to use during an assessment....just remember you can get a neuro on your patient just by talking to them (LOC = how is your greeting with them, and their response to you?...their facial muscles during speech, etc.) Skin assessment while listening to heart and lungs...

A lot of the assessment gets incorporated altogether, you will see this as you go! I remember being stressed out too during my first focused patient assessment, and now I do them on real patients in the hospital with ease.

Arrow Website help with physical assessment: "Screening Physical Exam"

Found this website tonight. I've posted it in a couple of other forums and didn't want to leave this one out.

http://www.meddean.luc.edu/lumen/meded/medicine/pulmonar/pd/contents.htm - the physical exam - this site is set up for medical students, but nursing students can learn from it as well.

This link takes you to a page of links to the various parts of the body to be examined. When you link into a body part you will find an explanation of the method to examine that site and what is normal, a button at the bottom labeled "Abnormal" will bring up information about abnormal findings during examination of that site. There are lots of photographs of abnormal conditions you can look at by clicking on highlighted text. The "Knowledge" button gives you a little outline breakdown of the pathophysiology going on. A fourth button, "Self Eval" is a list of test questions—remember these are designed for medical students, so don't feel badly if you miss a few. This is from Loyola University Medical School.

Explains how to examine the various parts of the body and has lots of photographs showing abnormal conditions and how to assess for them.

"Orientation is checked by determining if the patient can state their name, the day or date, where they are, and the purpose. This is documented as oriented times 4."
