I've been asked to chart a general nutritional assessment, and I have no clue where to start!! Please help!!!

When I took my physical assessment class, I remember we had to spend some time doing a diet recall. Is that what you are referring to? In our book there was a chapter specific to nutritional assessment -- that is what I would turn to.
