How to Take Notes?

I have about 2.5 months until I take the NCLEX-RN. I am using Saunders and I am enrolled in Kaplan Online. I would like to take notes as I complete questions and read rationals but, I am having difficulty trying to determine the best way to organize my notes (for review). Does anyone have any suggestions on the best way to organize?

I too had used saunders and Kaplan, I have some notes of my own, if you would like a copy please pm me, all the best.
