I came across this question in the prioritization n delegation book, page102:

what all will u delegate to a new nursing assistant? choose all that apply:

1. assist the pt. to bedside commode
2.chk pulse oximeter q shift
3. teach deep breathing n coughing
4. encourage to drink fluids

now the correct ans s r: 1 n 4

kaplan says that majority of the time in the statements like experienced LPN,NA .. the word experience is a distractor, my question is : do we really count the term new?

I wud have picked 1,2 n 4 as the right ans but in the rationale it says because it's a new NA u cannot assign pulse oximetry to them

can anyone explain that.......i thk it's so easy to miss out on anyone of the choices in select all that apply.......pls help

just to add more.... in the same book it says under a diff case study that the NA's can handle pulse oximetry........I know things vary a lot bsed on your facilities policies but who knows for sure what works for NCLEX?

the difference that I can see is you mention new NA. Therefore they may not be aware on what they are looking for with oximiter so should be delegated to someone who does. They should be able to transfer patient to commode and encourage fluids, where as pulse oximiter and teaching is more a trained nurse remit unless the NA has been taught properly and been assessed to correctly carry out those procedures

Kaplan strategies book says you can't delegate assessment, teaching and evaluations to NA or LPN. I would choose 1 and 4 because it's a standard unchanging procedure, new or experienced NA can do it.

I am an RN (passed NCLEX in July). I would have picked 1,2 and 4 as well. Even if they are not aware of what they are looking for, a new NA should be trained to check a pulse oximeter and record the result. It would be the RNs job to check results...Who knows w/ NCLEX though...it's one crazy test! I only got a couple delegation questions...mostly priority and teaching..you will do great!

The rationale the book gives is:

Assessing and teaching are not in the scope of practice of the NA. Checking pulse oximetry could be appropriate for experienced NA's once they have been taught how to use the pulse ox device to gather additional data.

I too would have picked it though.
