Head Injuries:
Concussion is a jarring of the brain inside the skull with temp. loss of conciousness.

Contusion is a bruise that can be subdural or extradural collections of blood.

Epidural hematoma is most serious type of hematoma, caused by arterial bleed, forms between dura & skull from tear in meningeal artery, emergency surgery needed!

Subdural hematoma forms slowly from venous bleed. Monitor LOC.

Subarachnoid heorrhage bleeding directly into the brain, ventricles or subarachnoid space.

For lumbar puncture position the patient lying on the side with the legs pulled up to the abdomen & with the head bent toward the chest. ( or as close to this as pts. flexability allows.)

Hypertension is persistent elevation of the systolic blood pressure above 140 mm Hg and a diastolic above 90 mm Hg. Is most significant predictor of developing coronary artery disease. Primary HTN = no known cause Secondary HTN = occurs as result of other disorders such as cardiac disorders, renal, endocrine, pregnancy or medications. May be no symptoms at first. Symptomatic usually = organ damage.

Suprapubic Catheters: Used to check for residual urine in bladder after voiding:
1. Clamp the S/P Cath
2. Have Client Void
3 Unclamp the Cath, allow it to drain to gravity, measure & document amt
***When residual urine is 75 mL or less, S/P cath normally removed***

Cleft Lip &/or Palate: Native American gravida, carrying a boy, most common instance

Post-Sigmoidoscopy: Dizziness and/or lightheadedness could signify hypovolemic shock r/t bowel perforation

Creatine Kinase (CK): Enzyme specific to brain, myocardium and skeletal muscle - elevated level indicative of tissue necrosis or injury (ex. acute MI) - Normal Range - Men 12-70 U/ML; Women 10-55 U/mL

This one I am not positive about.....please let me know which is correct:

Per Kaplans: When replacing trach ties, apply new ones prior to removing old to prevent dislodgment of the trach

My clinical instructor taught that you support the trach with your non-dominant hand while removing old ties and applying new ones with dominant hand

Not sure which is correct, or if either is technically incorrect....anyone know?

The Four Areas of Nutritional Assessment:
  1. Take a history of the dietary intake over the past several days to ensure the period extends beyond 24 or 48 hours. The diet assessment history gives insight into the individuals' daily routine, dietary habits, and patterns. Evaluate the diet to note areas of deficiency.
  2. Perform a physical examination of the individual to determine height, weight, BMI, overall hydration status, and general condition.
  3. Anthropometric (comparing measurements of the individual from previous exams) examination will establish normal growth and alert the health care provider to abnormalities in growth.
  4. Biochemical analysis or evaluation of the individual's molecular findings.

Immunoglobulins
IgA:
Found in body secretions, tears, saliva, colostrum and breast milk
Characteristics: lines mucous membranes and protects body surfaces

IgD:
Found in plasma
Characteristics: Present on lymphocytes

IgE:
Found in plasma, interstitial fluids, exocrine secretions
Characteristics: allergic/anaphylaxis, bound to mast cells

IgG:
Found in plasma, interstitial fluid
Characteristics: crosses placenta, secondary immune response

IgM:
Found in plasma
Characteristics: primary immune response, involved in ABO antigens


Neutropenic precautions
Monitor for signs of infection, strict handwashing, avoid invasive procedures, no fresh fruit or vegetables, no live plants or flowers, private room if possible and must keep door closed, avoid large crowds or crowded facilities

I am on the same boat..I am taking mine this Monday...and it feels like i am having a panic attack...anyways, i also felt like i didn't retained any of the things I studied...so, i am thumbing through my notes from kaplan...just a quick look and the more I read on my old notes, i start to remember things =) Anxiety meds won't help before the test. it will calm you but remember that they have a sedating effect...you might not be able to think clearly...try to do a deep breathing exercise just before the test...or, run (excercise the day before the test..not too much though so won't get tired the following day) and most of all...Let's all pray..it helps..goodluck!

to Joaniedee...yeah, i got that too...but according to kaplan, NCLEX follows the book and not the "real work place" ...what we have been thought at school (from Medsurg book: Ignatavicius) you slide the new one first then remove the old ties...I hope this helps =)
