Everyday life:
- Have you ever driven home when you were tired and realized after arriving that you couldn’t remember driving a portion of the trip?
- Have you ever found your cereal box in the refrigerator and wondered how it got there?
- Have you ever been spared a dead car battery by a beeping noise that reminded you to turn your car lights off?
Everyday life in health care:
- Anurse administers a wrong dose because medication labels look similar.
- A doctor is interrupted by a page and then gives the nurse incomplete patient orders.
- A nurse uses a new IV pump incorrectly because it doesn't work the way the old one did.
