misplaced license

I live in ohio, and my wallet got stolen. In it was my nursing license. How would I go about getting a new one? Has anybody had to replace theirs before? If so, how much will it be, what do I need to get it, and how long will it take? I am currently looking for a job, and it is essential that I get a replacement license.

This happens to the best of us. Contact the Ohio BON for direction. Be sure to let them know your license has been stolen so they can alerted to possible identity theft.

I lost mine a year or two ago, and if you go to the Ohio BON website, there is an affidavit you have to print out and mail in. My replacement license was $25. As an aside, I would not carry my nursing license in my purse. Just keep it at home.

Scroll to the bottow where it says "Affidavit of Lost Document." Follow the directions on that page.

I disagree with keeping the license at home. What if you come upon a car accident or something and need to verify that you are a nurse. Especially to prove that something you did was in your scope of practice.

 Thank you for your help and concerns. I called the state board and am in the process of getting a new license. I really love this web site it is very helpful. I am so glad to be a part of it. You guys are great!!I appreciate your replies. Sorry it took me so long to write back, but I am still learning how to.
