Post Triage Notes-forms from various hospitals

Hello all,

This is my first posting. I work at a University Hospital as a staff nurse. I am on a committee to change our nurses notes (triage, assessment, ongoing documentation, teaching..) We still use paper notes. Our hospital will be going to a computerized system, however, it is only in the 1st stages and will not be implemented for at least 2 years.

I was trying to obtain samples of nursing notes from different hospitals, to help us change and update our notes. If anyone could help, I would appreciate it.

We use the CTAS 5 level at this time and by March 1 are going live with E-Pod computer generated triage.

I can provide you with a scanned copy of what we use (paper) at this time, just p.m. me your email.

We recently went to an EDIS system, however everything is still paper. We have a triple fold nursing note that includes triage. We worked very hard on it, and it seems to work well. All facilities seem to reinvent the wheel, so kudos to you for looking for other resources.

Australia here,,, we use a computer first then that generates a form , which we use, then we use another form for the Assessment.
