Pepcid IV

After reading on a pepcid bottle it "for iv use only after dilution." Does everyone dilute pepcid or do you give IV push?

I have never given Pepcid IV push. Always dilute in 50ml NS.

I've always pushed it, and done so according to out reference book at work. Never even had someone say it stung- not that that proves anything.

I believe it is diluted in 10cc NS and given IV push. It has been a while since I have given it, we have pretty much gone to Protonix.

My drug book says:

Direct IV: Dilute 2 ml (10 mg/ml solution) in 5 or 10 ml of 0.9% NaCl for injection.

Rate: Administer over at least 2 min. Rapid administration may cause hypotension.

You might just be safer by calling your Pharmacy and making sure that you are following facility guidelines for any medication.

We hang it IVPB, with a special inline filter below the pump. If it is necessary to use the filter IVPB I would be concerned about using the drug IVP. The filter is packaged by and for the express use of Pepcid. It is diluted in 50 ml NS after first reconstituting with 5 ml sterile water for injection (per instructions on bottle). It may depend on the manufacturer and it's manufacturing process whether it needs a filter or not. I cannot remember if we have generic or brand name.

Pharmacy sends up pepcid separate and not from the pyxis, a 10 cc saline is included in the bag. But if you don't want to dilute it, just give it over 2 minutes. There shouldn't be hemodynamic changes if it's two minutes even if it is undiluted.

I've given it both ways. Dilution only means to dilute it farther than it already is, not that it HAS to be given piggyback.

Yes, it should be diluted with saline prior to infusion.
