Question forms

anybody know of a good place to look at various nursing forms available. My supervisor wants me to come up with a nurse to nurse reporting form.

You don't understand. I work for the navy. They want us to use SBAR and the navy doesn't often give options. I would much rather come up with something easy and concise to check off than to use the complicated and inappropriate SBAR. I've already made my own and submitted it for approval. We are still trying to get our trauma checkoff form approved. It usually takes about a year. What can I say, it's the
