Child Abuse Clearances

Filling out this form now to send off to Harrisburg...am I really expected to know my addresses since 1975? I wasn't even born until '80! I don't know where I lived when I was 2!

Sorry if this is a stupid question (my bf keeps telling me there is no way they could know its incomplete), I just know there is no way I can remember that stuff, I've lived in a lot of places. But I'm worried some how that it will come back to haunt me some way...

and everyone I've ever lived with...does this include college roomates??

If you weren't born until 1980, then there were no *addresses* prior to that to worry about, now are there? I'm sure your parents/guardians/ family members with whom you lived as a child can help you complete the information.

If college roommates received mail at the same address then yes, I would list them.

fill out as completely and honestly as possible and give explainations for any missing periods

Do I need to include addresses abroad?
Also should I include times I stayed with a relative for a few months between breaks or getting on my feet in this country?

Thanks for the replies, I just put everything I could remember. It didn't actually ask for dates to go with the addresses, so I didn't really explain gaps...well not gaps really, I just don't remember any addresses before middle school, so it goes '91-present. My mom is deceased, and no one else would know that info...

I'm sure it will be fine, I just like to worry about stuff.

Springgarden...I stayed with my mother in law for a few months when I first moved to philadelphia, and I did include that time...

This sounds pretty ridiculous to me, who is requiring this information? I was born in 1975 and although I could easily get my old addresses from when I was a baby (I figure I've lived at no fewer than 15 addresses), I have no CLUE what some of my addresses were or even who my roommates were! I don't have any idea what my first college roommate's first or last name was ... she was THAT unimportant to me! I lived with a boyfriend for 6 months once in the mid 90s and I can't remember the street address at all. Heck, I can't remember the street address from an apartment I lived in only 12 years ago... my husband just informed me of it, I guess I remember now!

Maybe my credit report would have all that info on it, they seem to have everything else!

The clearance is required before starting clinicals for school. The check is done by the Child Abuse Registry/Department of Public Welfare...

So I got the results back, clean of course. It only took a little more than a week, which was surprising to me, but no complaining

Hi,

Did it really only take a little over a week to get back? I need mine by May 1st and am scared to death I wont get it in time. I sent it last Friday priority mail.

Yup I mailed both (child abuse and criminal record check) via regular mail on Friday April 6th, and received both results on Monday April 16th.

But it could have come as early as the 12, 13th, or 14th, because I was out of town and didn't check the mail until Monday.

So start looking next monday!
