Question patient teaching forms . What kinds of forms do you use to document and assist

I teach associate degree nurses. We current use a form for students to document and assist in organizations of this information. It is a four column form with objectives, content, how to evaluate the teaching strategy, how to evaluate the learning of the patient. How do you accomplish this goal? Are there some fabulous forms our there ?

hi... im a beginning RN student.. and i need a form for patient teaching... please email me or reply here thanx! i will be checking tonight coz i need it by tomorrow at least.. for a rough draft... also what other teachings can i use for a psychiatric adolescent patient who is suicidal and has really bad temper.
