Legal Waiver for Supply Kits in Lab?

Hi...

I am implementing supply kits for practice for the students in my lab...but I have to write a legal waiver first. Anyone have any that their facility uses...that they'd be willing to share?

HI Kay, we have used skills supply kits for several years. I don't understand the waiver concern. Is it because the kits contain needles and other invasive equipment? I have had many students ask me if they can use some of the equipment on their family members. I always tell them that it is not a good idea because who knows about sterility integrity. Anyway, let me know if I can be of more help.

Hi...

Actually we have had two students (in two different colleges that I have worked at) who actually did use stuff on family members. One inserted a foley into a boyfriend...the other used an NG tube on (I think) a boyfriend. So...

We put smart tips in instead of real needles.
