For everybody who's given anesthesia so far; How many of you use lidocaine near the time of extubation and if you do, what dose and about when do you give it. My CRNA on thursday gave 40mg about 5 mins or so before the patient was extubated. It worked pretty well but I just wanted to hear some other providers experience.

If you are looking to blunt laryngeal reflexes so they don't cough, I usually give 1.5-2mg/kg about 20 minutes before I expect them to wake up. Works like a charm. They will be somewhat more sedated though. I don't do it with every case...just those that I don't want to cough and buck...i.e. cranis, etc.

YAY!!! One of my favoritist tricks in my little book!!! About 40-50 mg 5 minutes before i expect them to buck as they are getting lighter (about .5 MAC or BIS is creepin up), when i time it right, it works like a ******* CHARM, but its still an art, too soon delays the awakening, too late doesnt work. Im a newbee CRNA, but those seasoned CRNA's have perfected it, still workin on it!

I consider that it worked, when they are laying perfectly still and I call their name and they simply open their eyes and look at me!
