End of Life Issues

Hi,
I'm a graduate student doing a study on nurses and physicians attitudes and experiences with end of life training.

The survey is 10 questions long, and will take a maximum of 10 minutes (probably less) to complete.

I need to get a statistically significant sample of nurses who have direct patient care of any kind.

Any nurse who fits this description who would be willing to complete this online questionnaire, please email me at eolsurvey@yahoo.com and I'll send it to you. All you need to do is click on the attachment (it's a word form), complete it, save it with any unique name you choose, and email it back to eolsurvey@yahoo.com.
