Nursing Flowsheet

Hi,
I just recently accepted a position in a Cardiologist office. I have never been an office nurse before. I love it! The physician I work with currently jots down info on a blank sheet of paper. I am not comfortable with that.
I am looking for a quick flow sheet that I could chart on every visit. I would like to compare vital signs from the last visit, verify meds etc. A place for notes would be nice. I don't want a legenthy admission sheet.
If anyone has one they could scan, or direct me to a site that has samples, I would be forever grateful.
