In the nursing home where I'm employed, med errors are a very common occurrence. Typically, it is because the OTC med (ferrous sulfate, niacin, etc.) is unavailable. Also, nursing homes do not have in-house pharmacies, and are at the mercy of inefficient offsite pharmacies.

so the ltc facility orders these meds in a timely manner?
and it's just the pharmacies that are late?

I've made a couple of minor errors but I've reported them to the super and the doctor and the patient.
