Talking How to be a better patient/nurse

I'm doing a bit of research for a project I'm working on. The first part inevitably requires asking how can we be "better" nurses for our patients. I decided to look at the other side of the issue as well by asking How can our patients be better patients for health care providers? For example having a list of current medications prepared before they get sick to bring to the hospital or being more active in their care. I would like to hear responses to either one of the questions....professionally formulated or even just what drives you nuts!
Also, if you have been a patient yourself, I would like to hear your stories...your impression of the care you received NOT as a nurse-patient but as a patient receiving nursing care.
I've been in that situation with major surgery 5+ times and have plenty to say about the quality of the nurses from a patients perspective...some that I had worked with for years.

They need to be involed with their discharge planning for day one. Dr may tell them they will be there for 5 days. Insurance may say something else. They states they can not go home because family has something to do and the next day will be better. It is hard to get insurance to give us another day.
