I am a home health nurse who has a patient with a 3rd degree burn on his back. He is elderly and has other health issues that are impairing the healing process. I would appreciate any advice or suggestions on wound care (it is an area approx. 2.5 cm x 3.0 cm with red hypergranulating tissue.) The current wd care is nss cleanse-panafil spray-dsd change qd. Thanks!!

Keep him off his back and air circulating around the dressing as much as possible. Unless contraindicated, encourage high protein drinks.

As our WORN has taught me, RED granulating tissue = infection, especially if hypergranulation is present!! I suggest you try Acticoat or Actisorb (if there's a lot of discharge), burn dressing, or maybe get a script for Bactigras gauze dressing. Has a swab been done? Burns often get pseudomonas in them, in which case acetic acid (or vinegar) is the solution (no pun intended!)

** IMPORTANT NOTE: if you're using acticoat or actisorb, you must NOT use NS. This will inactivate the silver on these dressings. They must be used only with sterile H20. Dip the dressing in the water before applying to the wound.
