consent form for NSAID tx

I am currently working on changing a policy at my facility RE: NSAIDS. We've had some problems with abuse and our providers not documenting that they've gone over side effects of the medications with the inmates. One of the suggestions is to have the inmates sign a consent form that explains the possible side effects and then placed into their chart. Does anyone out there already do this and if so would you be willing to share the consent form for me to present to my director? Thank you in advance for your help.

At my institution, everyone that is on NSAIDS gets a side effects and drug information packet and we have to document that we gave it to them. I like the idea that the inmate has to sign something saying they understand the treatment. Maybe you could just have an information packet with a place to sign at the end? Or they would have to sign a form saying they read and understood the drug information packet that was given to them?
