time out documentation

In our radiology department ,there is some confusion, as to which procedures require the time out documentation. Is anyone using them for Barium Enema's, UGI's, or how about Hysterosalpingograms?

We require a "green sheet" which is how we document our facilities time-out process on any procedure that in considered invasive, aside from the administration of Contrast. We would also include any procedure that potentially has laterality.

So in your situaion, we would definitely include hysterosalpingograms, because aside from being diagnositic, we often intervene if a narrowing exists.

Although we verify order, patient ID and procedure for UGI's and Barium enemas, we do not typically perform a time out. Althogh I do suspect we are moving toward that direction soon.
