Who does a "Community Not for Profit" hospital answer to?

It seems long ago in nursing school I was taught: when a hospital is designated as the "Community Hospital" and it is Not For Profit and is the only hospital in the community that the board of trustees are answer able to the people of the community. It is not owned by the community but it is answerable to the people of the community as it is the only source of hospital care within said community.

Am I off base here? Can anyone help to clarify with factual information?
