What current health care/practice issues in this specialty?

I didn't know so many types of specialty nursing fields existed. I'm new and I've read some threads on forensic nursing already. I see that the role and responsibilities are explained thoroughly. What I am interested in knowing is what current obstacles or issues exist in this specialty? Health care or practice issues?
