You can attach your by clicking the paper click link on the reply page (next to the smiley face. Then you put the file location on your computer in the browse field and upload it. Look forward to seeing yours as I'm a brand spankin' new RN.
I pasted mine above, right after the one i posted asking about your wishlist.
I made it myself, though i'm not 100% satisfied with it.

Here's one that I use. It's for the night shift and you can change the times if you are days. I fold the sheet in half for two patients.

Here is one, will hold info on up to 6 patients, you can tweek it as you like, I draw columns on the back side and place pt labels and write down all PRNs throughout the shift.
