			<content:encoded><![CDATA[<div>Hi everyone, I know that there are a lot of agencies out there.  I don't know the interplay between the agency and the health care entities, but I assume that the entity pays the agency either a flat rate to provide them with labor or they bill them per nurse and then give that nurse a portion of the money.  Either way, they're paying an agency some overhead.  Would it be feasible to talk to local hospitals and offer personal service when called, if available?  You wouldn't have to be an actual &quot;employee&quot; as if you were per diem, and I assume that the hospital would be able to pay less to hire you directly then to hire agency or even some per diem rates.  You would, of course, show the hospital all your certifications.  Then the hospital could pay you directly with a check and a 1099.  Has anyone done this?  Is this practical or am I missing something?  <br />

It seems that there is a great need for this because my clients are always asking me to provide foot care for them. Due to this economy I really do need the extra work especially before Christmas. <br />

It's something that has interested me as well, so I was wondering if it wouldn't be easier to just open a staffing agency, where you could refer yourself to a home care agency and ask for a certain amount per visit. Of course it would be higher than they would normally pay since you would be bringing in your own clients. Any thoughts on this? Are there any medicare rules/reg pertaining to staffing agencies? <br />

I currently work for both a staffing agency and a home care agency. I have the possibility to generate medicare clients (from established connections with dr's, assisted living, etc), but don't want to work as a marketer. I would prefer to be compensated for both bringing in clients as well as making the home visits. I also know that its illegal to get compensated for referring clients (ie per head), so I'm trying to find a good medium (and legal way) for doing this.
