No surprise here. This is just from one county in the US where the medical examiner has taken the time to explain why the dx was not made earlier. His report is very helpful in understanding this phenomenon.

We've been noticing and commenting all along that some cases were only diagnosed at autopsy.

As swine flu was spreading around the globe this spring, a senior disease specialist from the World Health Organization (WHO) held an urgent conference call with top British health officials. In the conversation this May, later described as "aggressive" by sources familiar with the discussion, the WHO official accused the British of concealing the extent of their country's swine-flu outbreak. Among those with swine-flu symptoms, Britain was only counting people who had traveled to places that, like Mexico, had already confirmed an outbreak of the virus, known to scientists as H1N1. Their method left much to be desired in a country where the virus was already spreading fast. Countless Britons fell sick and were intentionally left uncounted.

Governments, of course, have a long history of concealing outbreaks, and this year's flu pandemic, while the first of this particular century, was certainly not the first to be brushed under the rug. The consequences of cloaking swine flu weren't disastrous on this occasion, but the result will not always be so benign. In fact, at this very moment, another virus -- with the potential to be far more devastating -- is continuing to seed infections, frustrating efforts to root it out. That virus, H5N1, or avian flu, is a far more lethal strain. And you guessed it: front-line countries' records in candidly reporting the disease's spread don't bode well.

If there's one thing past pandemics have taught, it's that curing the world of flu is impossible unless countries are upfront about their outbreaks. Armed with that vital information, health officials can take steps to slow the spreading infection and, if containment fails, ramp up emergency medical care and other vital services. Without timely disclosures, it's much harder for virus hunters to discover how an emerging disease attacks its victims and transmits to others; it's also much tougher to get virus samples for study in the lab.

In my area of the world, (maybe other places too?) our seasonal flu vacc has been in dire short supply. I had flu vacc clinics scheduled throughout October and November, but had to cancel because we had no vaccine! All our neighboring hospitals and public health are in the same boat. It seems the seasonal flu vacc is delayed because the manufacturers were busy producing H1N1 vaccine. So my fear now is, could it happen that the seasonal flu will become the over-arching nemesis? The manufacturers keep promising we'll get our full shipment, but by then, what if it's too late?

I had swine flu. It is almost a badge of honor, suggesting that the speaker survived the first pandemic of the 21st century and is immune to the next wave.

It also may be wrong.

Tests at Children's Hospital of Philadelphia suggest that large numbers of people who got sick this fall actually fell victim to a sudden, unusually severe - and continuing - outbreak of rhinovirus, better known as a key cause of the common cold

Neither the federal government nor the states track rhinoviruses in the way they do "surveillance" for influenza, based on samplings of doctor diagnoses, emergency-room visits, and lab reports. Children's Hospital of Philadelphia is one of the few institutions that routinely checks for them whenever it tests for influenza and other viruses.
Rhinovirus - named after the Greek word for nose - is known to circulate year-round, and typically to peak shortly before and after flu season. Children's recorded rising numbers in September, right on schedule. Then they kept rising.

"The rate of activity was unbelievably high," Richard L. Hodinka, director of the clinical virology laboratory, said yesterday. "What got my attention was not only the numbers we were seeing in the laboratory, but physicians saying there was severe disease."....

...Besides the sheer numbers of rhinovirus, Coffin was surprised that it was causing more problems - wheezing, pneumonia, fever, and lower-respiratory-tract infections - than are normally associated with the common cold, which typically infects the upper respiratory tract. That has led her to suspect that a strain not seen here before may be responsible. The CDC's lab will attempt to identify the strain....

...And doctors in Louisville, Ky., at least, noticed a similar phenomenon: serious illness (in both children and adults) that did not test positive for influenza. They, too, were surprised to find rhinovirus.
"We haven't thought of it as something that causes kids to be really sick and need to be admitted to the ICU," said Kris Bryant, hospital epidemiologist at Kosair Children's Hospital....

I know, yet another vaccine! But this is actually a very good idea if you happen to be in one of the risk groups for complications as I am. Dr. Schuchat is sending out this letter to providers to urge them to vaccinate those of us that are at risk for pneumonia should we come down with flu.
During influenza outbreaks, pneumococcal vaccines may be useful in preventing secondary pneumococcal infections. As you know, PPSV is available for prevention of pneumococcal disease among adults and children 2 years of age and older who are at increased risk for pneumococcal disease, but many of those with indications have not yet received the vaccine. A 7-valent pneumococcal conjugate vaccine is recommended for all children <5 years of age.

CDC’s Advisory Committee on Immunization Practices (ACIP) recommends a single dose of PPSV for all people 65 years and older and for persons 2 through 64 years of age with certain high-risk conditions (Box). People in these groups are at increased risk of pneumococcal disease as well as serious complications due to influenza virus infection. A single revaccination with PPSV is recommended at least 5 years after initial vaccination for people 65 years and older who were first vaccinated before age 65. A single revaccination is also recommended for people at highest risk of disease, such as those who have functional and anatomical asplenia, and those who have HIV infection, AIDS or malignancy and have at least five years elapsed from receipt of first vaccination.

All people who have existing indications for PPSV should be vaccinated according to current ACIP recommendations during this 2009-2010 influenza season, which will likely include circulation of both seasonal and 2009 H1N1 influenza viruses. Special emphasis should be placed on vaccinating adults under 65 years of age who have established high-risk conditions; PPSV coverage among this group is very low and this group may be more likely to develop secondary bacterial pneumonia after an influenza infection. Use of PPSV among people without current indications for vaccination is not recommended at this time.

We have posters all over the hospital with propaganda about how the facility cares about patients and the community, that is why staff who did not get the flu shot due to medical or religious reasons have to wear a mask. WOW, talk about corporate taking whatever they can to advertise on fear. No where in the poster does it specify that the flu shot is the regular flu shot, not the H1N1 or both. The community has no idea what kind of flu shot is manditory, in their mind they are probably thinking that it's H1N1.

Now the coporate is posting the names of the people who should be wearing the mask by name in each department. This is awful... What next? What about our human rights???

Tests show fall outbreak is rhinovirus, not swine flu

Not that they weren't seeing swine flu, too, but doctors at Children's Hospital of Philadelphia...

Not that they weren't seeing swine flu, too, but doctors at Children's Hospital of Philadelphia were surprised how many tests came back positive for rhinovirus, not swine flu, in their patients that presented with wheezing, pneumonia and lower respiratory tract infections:

http://www.philly.com/inquirer/home_...html?viewAll=y

Excerpt from story (full story at link above):

I had swine flu. It is almost a badge of honor, suggesting that the speaker survived the first pandemic of the 21st century and is immune to the next wave.

It also may be wrong.

Tests at Children's Hospital of Philadelphia suggest that large numbers of people who got sick this fall actually fell victim to a sudden, unusually severe - and continuing - outbreak of rhinovirus, better known as a key cause of the common cold.

Experts say it is logistically and financially impossible to test everyone with flulike symptoms. And signs, treatment, and prognoses for a bad cold and a mild flu are virtually identical, so the response hardly differs.
But the finding may send an important message to parents who (despite doctors' recommendations) are questioning the need to immunize their children against swine flu because they seemed to have already had the disease, said Susan Coffin, director of infection prevention and control at Children's Hospital.

"Maybe their child is still susceptible to H1N1 and should still get the vaccine," Coffin said.

For years, rhinoviruses have been the Rodney Dangerfields of microbes. Even major institutions have found plenty of reasons not to pay them much mind. They are exceedingly common, they cause mere colds, they come in hundreds of hard-to-identify strains that make testing a challenge, and there is no effective treatment anyway.
