Do any of you aesthetic nurses perform microdermabrasion and then a chemical peel right after? I had heard of plastic surgeons doing this, so I tried it on myself and two other people. The results are so much more effective than getting them done separately at different times and seems still to have no danger or side effects since peels and micro work in two different ways. When I trained at AEI, we did peels on each other one day and micro the next, so apparently, Barbara Jones (the instructor) doesn't have a problem with them being done together either. Let me know. I was thinking I could promote getting them both done at once and give a discount.

Depending on how agressinve you are it may increase the risk of hyperpigmentation especially in skin type 4 and above. Are you placing patients on melanocyte supression prior to treatment if so this risk is reduced.

Another neat trick is to use an ultrasound in combination with peels to increase penetration due to electrophoresis.

thanks for your advice. I forgot to mention that I have not/will not do the combination of treatments on type IV and above. What exactly is melanocyte suppression? They did not mention that term in my training

If you want to supress melanocytes there are some rx and otc products availiable. hydroquinone 4% is an rx i write a lot, 2% is otc. Kojic acid also can work if someone wants a more natural treatmenty.

If you place skin type 4 on this with sunscreen for a month it reduces the risk of hyperpigentation. This is also benificial if used in conjunction with melasma treatments.

The reason I took it off the menu is that regardless of skin type you can only go to about a 40% glycolic or 2 coats of Jessner's in combination with 3 passes microderm. Going higher results in (99% of people) getting what I call cat scratches. It's the streak marks of the microderm that turn into scabs due to the peel. The pt. will have a week or so down time. When healed the skin looks great.

The problem selling this is that you can't give the person stronger peels (70% etc) so they don't get maximal benefit and start complaining why your not giving them stronger stuff. In order to get a stronger peel they have to give up the microderm. Well my price for a combo tx was $150, a single peel $110 and one microderm $99. They get a deal on the combo tx and don't want to pay $110 for only a peel so they just complain. OR the person will want you to break up the peel and microderm into 2 sessions in order to get the stronger levels. Well that means your now getting double the work for half the money. If you say no, they leave your practice thinking your a rip off, For you it's a no win situation.

I have tried offering the $150 deal with full explanation that they could only get up to a 40% glycolic in combo. Pt. understood, signed consent stating explanation and in the end still complained and wanted to move up to the stronger stuff but not wanting to pay. Eventually, I got so sick of it I stopped offering it. Now I just say you can do micro one week and peel the next.
