Looking for information on becoming an RN IP in Texas

Hello All!!!

Please provide me with as much information as you can. I am moving to Texas in Sept '12 and need information on becoming an RN independent provider in Texas. i am currently an IP in Ohio.

a)Where do I find the Enrollment Packet
b) How much do RN IPs get paid per visit
c) How do you find clients
d) Third Party Medicaid billing company (in ohio I used Rhinobill.com)

Will greatly appreciate hearing from current RN providers in TX to give me insights.
