How to do skilled visits independently and bill Medicare

Hello....I am seriously doing some intensive work on transitioning out of my current situation in geriatric care. I absolutely hate it and since I have been in it for about 4 plus years all of the resumes and applications that I am putting out to the hospitals seems to get ignored. I am feeling that the hospitals think that geriatric nurses are not "worthy" or something, but after 13 years in the hospital and working in Critical Care with my CCRN previous to the geriatric stint, I do feel that more attention should be given to me. However, maybe this is a nice little sign that I am being sent to finally do "my own work" and move on. I recently did start my holistic health-wellness business this past Fall, but it is not enough currently to do that on a full time basis.

I also love to teach, all sorts of things with holistic health and disease process, but do not have a BSN and no desire to pursue one.

I am wondering if anyone here would know how to get involved with doing independent skilled visits and then bill Medicare as allowed? I know that there are some home care agencies that do this and pay RN's like 45 per visit. I have researched and heard that Medicare pays them at least 100. So you can see how beneficial it would be to x out the middle man and do my own thing. I do realize that I would be responsible for my own documentation and supplies, although I think most supplies should be covered FOR them through Medicare.

Any advice is Greatly appreciated! I am so ready to get out of working for others!:D

Is Calif the only state that lets an individual nurse bill medicare for services?

i also want to know about any texas indy providers. have there been any that have started a home agency. i heard there is a new rule saying you have to have 80k in the bank before they let you run one now. is that true? i haven't read elsewhere about that so i'm skeptical.

some one...anyone....from texas....let me know what's the deal.

also let me know how long it took to start up after the requirements. how long before finances start coming in, and how many employees you have?

Isnt medicare a federal program with the same rules in all states? I am in NY and wonder if an RN in NY can independently bill medicare?

I don't know about other states, but in WI. we, as independent nurses certified with the state, providing private-duty, don't bill Medicare, only Medicaid.

I'm an Independent Nurse Provider (RN) with Medi-Cal (California's Medicare). In California, you do not need to be a NP. I needed a certain number of hospital hours in my field (Peds) in the previous two years in order to apply for my INP #.

Medi-Cal needs a Plan of Treatment (POT) signed by the MD and a Treatment of Authorization (TAR) for a patient, and they then give that patient a certain # of billable nursing hours. You take care of the patient, and bill Medi-Cal. About 500years later, they'll pay you (just kidding - but, plan on 6 months from starting the paperwork for your INP to getting your first paycheck - absolutely NOT kidding about that!)

Well IM an LVN and I just submitted my application to Medi-Cal for my provider number,

I formed a Corporation that I will be working under I have my NPI,insurances business address phone lines,medical supplies etc. etc.but in the meantime while waiting on my approval from medi-cal...Im learning how to fill out and submit TAR request and making sure I have my RN in place to do my case managment,

But what Im looking into now is providing care to medi-cal and medicare patients will be great but how do I tap into that private insurance or that no insurance market A.K.A. fee for sevice patients...any advice would be appreciated

Thanks for all the info so far. Im in the same boat, wanting to start my own home healthcare service as independent nurse contractor. I have also submitted the required docs to be Medicare certified. To be built into the system of the private insurance companies, you need to submit claims to them. They will pick up the cost left from Medicare. I'd like to know more also on POT, TAR and sample billing to Medicare. Everything is still in grey area with me. Thanks a lot for the help.

kakalinarn question for you, how do you become medicare certified???
