Evidence of swine flu risk to pregnant women rises; experts urge early treatment

Remember that the CDC is now recommending special considerations for pregnant HCW. Most hospitals and health care facilities have not yet implemented those guidelines. They might not if no one mentions this to Risk Management. Feel free to copy this article and show it to RM with the CDC guidelines. I am going to be doing this also.

If you are pregnant and you get sick, take the Tamiflu. It is safer than not taking it.

There are mounting and troubling signs that swine flu and pregnancy don't mix well.

Six pregnant women in Manitoba are reportedly on ventilators because they are severely ill with the virus.

And at least two pregnant women in the United States have died of swine flu complications after delivering babies by C-section.

A pregnant teenager in the Dominican Republic died, as did a pregnant woman in Scotland.

A woman in St. Theresa Point, a First Nations community in Manitoba, miscarried after contracting swine flu.

Humankind's relationship with the new swine H1N1 virus is still in its infancy. But people who've studied the issue of pregnancy during flu pandemics don't like the signs they are seeing. Dr. Denise Jamieson, an obstetrician-gynecologist with the U.S. Centers for Disease Control's division of reproductive health, says she finds the evidence to date "very unsettling."

"I am concerned about this," Jamieson said in an interview from Atlanta.

"There does seem to be increased severity in pregnancy. We don't have hard and fast numbers but there are enough reports that are concerning."

Data released by the CDC last month said at that point, 17 per cent of Americans hospitalized for severe swine flu infections were pregnant women.

A report a couple of weeks back in the World Health Organization's journal, Weekly Epidemiologic Record, noted of 30 swine flu patients hospitalized in California, five were pregnant women. Of those, two developed severe complications - spontaneous abortion and premature rupture of membranes.

...the fatality rate was higher in pregnant women during the 1918 and 1957 pandemics, though not the milder pandemic of 1968.

"If we base it on what we know of the 1918, 1957 pandemics, what we know about pre-existing antibody levels to swine influenza in the population, based on that I would say for this particular virus, pregnant women may suffer more serious consequences, especially in the third trimester," she said.

"And they should probably seek care early if they have influenza-like illness."

Studies done after the disastrous 1918 Spanish flu - which took its heaviest toll on young adults - showed astonishing death rates among pregnant women, said Dr. Michael Osterholm, an infectious diseases expert at the University of Minnesota.

Skowronski's review paper suggests there were also very high rates of spontaneous abortions during that pandemic - 26 per cent in pregnant women who became infected and 52 per cent among those who went on to develop pneumonia from their infection.

Osterholm explained pregnancy is a precarious state for a woman from an immunological point of view. In order that the mother's body does not reject the fetus, part of the immune system has to be effectively dialled down.

Other factors are also believed to come into play, including reduced lung capacity, Jamieson added.

She said that while the CDC doesn't yet have firm numbers, they are hearing that some pregnant women are reluctant to take antiviral drugs when they are diagnosed with swine flu. In some cases, their physicians share the reluctance.

Jamieson said given the risk swine flu poses to pregnant women, any who feel they may have contracted it should seek care quickly and should tell their doctor about potential exposures to people who had the virus. And they should take the antiviral drugs, she said.

"The message we're trying to get out is: 'Don't delay. If you suspect influenza, initiate antiviral therapy appropriately even before you get the testing back," Jamieson said.

"We definitely feel like in a situation like this, the benefits outweigh the risks of giving antiviral medication."

A pregnant woman from Brunswick County, N.C. died from swine flu complications after giving birth last week.

The woman, whose name will not be released, died in Durham, N.C. at the Duke University medical center, according to Brunswick County Health Department officials.

Fred Michael, deputy director of the Brunswick County Health Department, said the woman went to the emergency room at Brunswick Community Hospital and tested positive for swine flu about a month and a half ago. She was treated for swine flu and transferred to a Wilmington area hospital to try and help the baby continue to develop to a healthy birth weight and condition. Eventually she was transferred to Durham, where she gave birth some time in the last two weeks. Michael said he was not sure how soon the woman gave birth before dying from complications. He said to his knowledge, the baby was born healthy.

Public Health Minister Witthaya Kaewparadai yesterday ordered chief health officers in 11 provinces in the upper central region and lower North to strengthen flu screening measures, particularly of pregnant women.

The minister said every pregnant woman visiting a state hospital should be screened for the influenza virus. All would be asked if they have flu symptoms.

A pregnant woman died last Friday in Sukhothai after suffering from a fever for six days. She did not seek medical help, Mr Witthaya said.

Monica Rodriguez, 27, who was 5 months pregnant with her second child, came down with a severe cough and fever that was ultimately diagnosed as the H1N1 flu. Rodriguez, who lived in El Monte, died Sunday.

Gonzalez, speaking to NewsCentral's Suraya Fadel in Spanish, said Rodriguez was in and out of several local clinics, but her condition got worse. Gonzalez took her to Methodist Hospital in Arcadia, where she was rushed into intensive care.

Just days after being admitted, Gonzalez and her unborn baby died.

I don't understand this diagnosis.

A 25-year-old Roseland woman died Friday in part from the H1H1 virus, authorities said.

Carrie Brinson, 25, of 10233 S. King Dr., was pronounced dead at 1:25 a.m. Friday at Rush University Medical Center, according to the Cook County Medical Examiner's office.

A Saturday autopsy determined Brinson died from H1N1 influenza, pneumonia and intrauterine pregnancy, according to the medical examiner's office. The death was ruled natural.

I suspect the reporter copied portions of the official autopsy report. Maybe the autopsy report listed "intrauterine pregnancy" (in an official manner) to distinguish the pregnancy from an ectopic pregnancy.

That's pretty standard OB-speak, often abbreviated 'IUP.' Pregnancies can establish themselves elsewhere, and especially in a pregnant woman's death you'd want to know that the pregnancy was intrauterine and not ectopic or abdominal.

Brinson lost his wife Carrie and their unborn child Friday night. She was 5 months pregnant and they were looking forward to starting a family after four miscarriages.

A couple of weeks ago, Carrie came down with flu symptoms and after a few days she was taken to Rush University Medical Center where tests showed she had the H1N1 virus.

Brinson says his wife didn't receive the vaccine because he says it wasn't available.

Susan Ford, 31, was 35 weeks pregnant when she was admitted to hospital with severe, flu-like symptoms.

Her condition deteriorated rapidly and she was given an emergency caesarean section early the following day.

Staff delivered a healthy daughter, Poppy, but Miss Ford's condition became more acute and she died over the weekend, just 18 days after becoming a mother.

Yesterday her devastated family questioned why she had not been treated on a special blood oxygenating machine.

This is a strange story. Is this a relapse of flu? We know that many people do start to get better, and then suddenly get worse because of the onset of a pneumonia. They don't tell us how long she was out of the hospital before being readmitted with pnemonia.

A Silver Creek woman, who was forced to give birth early after being diagnosed with swine flu, was recently hospitalized again after suffering a relapse, her mother said.

Genita Magee was almost eight months pregnant when she started suffering from cold-like symptoms. Magee was hospitalized and her symptoms continued to worsen, her mother, Dorothy Lenoir said. Doctors were forced to deliver her baby almost a month early.

After more than a month in the hospital, Magee was allowed to go home. But late last week, she was rushed to the hospital because she stopped breathing, Lenoir said. On the way there, she was involved in a wreck. Magee wasn’t injured in the wreck, but she is back in the Intensive Care Unit at Baptist Hospital in Jackson being treated for pneumonia, Lenoir said. She also has fluid on her lungs.

St. Mary's has reported four confirmed cases of swine flu to the health department...

Only one of the four - Jennifer McGarrh - remained in the hospital Sunday. McGarrh was pregnant when she was admitted in September and is still listed in critical condition, though she is no longer being treated in the Intensive Care Unit.

Indigo girl: You seem so willing to share all of your knowledge about H1N1, can you PLEASE guide me to some information on Pregnant women recieving the vaccine vs. not recieving the vaccine. What are the risks of taking the vaccine vs not taking it. I was soooo against the vaccine two weeks ago but now and rethinking it because my daughter just found out she is 7 weeks pregnant!!. I am not asking for medical advice just knowledge to help her make a decision on weather she will get a H1N1 vaccine. I am just searching for educated nurses mostly because you all see this up close. From what I have seen the propaganda against the swine flu vax is not founded on any proof. If there was any, someone would have surely posted something significant by now yet all we have seen are links to sites with no scientific research or proof. It is a mystery to me what all of the suspicion is about.

I had not yet heard of any adverse effects for pregnant women with this vaccine, but it just came out. In its favor, is that it is exactly the same as the seasonal vaccine except that is a strain change. The testing has been done to determine best effective doses for the different groups of recipients. Nothing else is different.

I have been seeing a consistent effort to try to link this vaccine with a vaccine produced for a completely different virus over 30 years ago. It makes no real sense. Why would anyone believe that there is a link when it is not even the same virus? The 1976 virus never even went pandemic yet here we are with a real pandemic on our hands, killing

otherwise healthy pregnant women and little kids, and people are going to pass up a vaccine that is specific for this strain because they are afraid of what happened over 30 years ago? This defies all logic.

This novel virus can be very dangerous for pregnant women. I started documenting cases early on because of the well known history of how pandemic viruses target pregnant women, and right from the first American case, it was apparent that this was novel virus was going to do this also. Your daughter should talk to her health care provider and make her best choice. I hope that she chooses the vaccine because not only will she be protecting herself but also that baby. I am heartsick about the deaths of all of these lovely young women. Seriously, it tears me up to read each case. There is no longer any reason for them to continue to be at risk in this random roll of the dice that the flu is performing. No one knows who will be unlucky, but it seems foolish to tempt fate when there is a good alternative. Not every pregnant woman with swine flu will be adversely effected, but we don't get to know who will in advance. If your daughter is worried about thimerasol, she can get the single dose syringe option without it.

No vaccine or medication for that matter, is totally without risk, but I personally do believe that the benefits outweigh the risks for this vaccine, and I am extremely gratified that it has finally arrived. So what are the risks? The same risks that there would be for any vaccine. Some are worried about Guillan Barre again because of 1976. GB can occur naturally as a result of getting viruses, and rarely has been associated with some vaccines. Will the shot give someone GB? I don't have a crystal ball, but would there be any greater risk than with the seasonal vax? Probably not, because this

is only a strain change, and everything else about the vaccine is the same. The idea that it might because it happened in 1976 again defies all logic. There is no connection between the two vaccines or the two viruses. The 1976 swine virus died out. It has never reappeared, and if anyone thinks that thinks that it has, they should be prepared

to back this up with credible evidence which of course, does not exist.
