CMS releases new proposed Prospective Payment System

Hello All: I was wondering what you thoughts were on the new proposed Prospective Payment System for dialysis faclities. There is much to be concerned about and after listening to the open-door forum that was held by CMS the other day, I wonder if we are not going backwards versus forwards. I would love to hear your responses.

Not very educated on the matter. But I've heard the term "bundling" quite a bit. Sounds like the dialysis companies will be loosing money. Working in Home Therapies, we are being pushed to admit new patients. Keep hearing the term "grow home therapies" repeatedly. I'm thinking the dialysis company will be paid the same for incenter or home patients. A lot of the home therapy nurses are being stretched very thin right now. :twocents:

If you are in home dialysis you should familarize yourself with the Proposed Rule. The bundling will affect home dialysis --- how??????? Presently home dialysis training is billed separately. With the bundling, the training will BE INCLUDED in the bundled (fixed) amount that providers will receive. The first four months of treatment, facilities will receive 47.3% increase, however, most patients do not start home until well after the fourth month, therefore, facilities who do not provide home training will benefit from this increase. I would suggest that you and other home nurses familiarize yourselves and write a public comment. Providers should be educating their staff and encouraging them to write public comments but I guess the providers feel they have the congress people and CMS in their back pocket. It is rural and independent facilities that are going to lose but more important is the patient who will lose................if you want more infomration you can go to www.renalweb.com and scroll to the bottom. There is a full series of data, information regarding the bundle. Then you can go to www.kidneypublicpolicy.com to obtain more information and even post questions.
