How often do you change disposable inner cannulas? Is the policy in your facility based on evidence, manufacturers recommendations (can be self serving) or just a product of "that's the way we have always done it"

we change inner cannulas q4 hrs for the first 72 hours postop and then do then 3x daily and PRN depending on secretions. not sure what the policy is based on but it's working
