Need help with a nursing skills checklist


I am in the process of trying to come up with a skills checklist for the nurses in the medical dept of the regional jail that I work. Our nursing staff consists of 3 RN's and 10 LPN's. My HSA and I were discussing one day that it seems like some of us have let certain skills slip since we do not perform them on a regular basis and there are certain staff members that will suddenly disappear when certain tasks need to be performed because they don't know how to perform the task. So, I opened my big mouth and now my HSA wants me to write up a skills checklist and each staff member will be signed off once competency is demonstrated. I need help with creating this list please!! Any suggestions would be greatly appreciated! Thanks in advance!

Why do not buy a book from fundamentals of nursing skills check lists where everything is spelled out?

The list will be very long.....and I really think it should be things that apply to your facility, not anything that you guys would never come across.
Here are some examples of ours....

Reading manuals (P&P, disaster manual, infection control manual, etc)
take computer class
new employee orientation class
meet with scheduler to learn how our schedule works
meet with Pharmacy (pharmacist gives 20 minute overview of the system)
meet with Dental
Meet with mental health

Our staff has to have each item witnessed 3 times and signed off
glucometer
PPD
vital signs
wound care
patient teaching - crutch walking, etc
labs - listed individually and must be witnessed and signed off
how to use certain forms (the forms are listed, like for housing change, etc)
Our book is about 8 pages long (probably because all labs, forms and each task are listed separately)

We also have a scavenger hunt when they first start.
