CNA cover letter, question?

1-Can you give me an example for the CNA cover letter's TITLE?

2-For the CNA resume, under the Education section do I just put my CNA school or do I also include my High School and College here as well?

For the resume, we were told to put your high school only if you graduated in the past 5 years. If not, leave it off. And college info, no matter how long ago it was, should be included!
