Breathe! I'm assuming you've taken tests from your instructor in this time and passed. You've practiced your skills. You have your book and this site if you have questions.
For the skills test remember that handwashing is super important to them along with removing gloves. When I took my state test I mentally sang "Happy Birthday" when I washed my hands because that takes approx 15 seconds to sing. If the state rep asks you a question think before replying. Do not be in a hurry. Be methodical.
It was 6 years ago I took my test. I had to take it on a Sat and I remember being so nervous that I couldn't wait to get it over. So I told the state rep that I had to be in a wedding later that day and could I possibly go first? I really did have to go to a wedding but I wasn't in it. She laid out cards and we had to pick 3 skills to perform. I got super super lucky: back rub, brushing dentures, and ROM. I hope you get as lucky as I did.
I don't know if the written tests vary state to state but I do remember that it was very straightforward and no trick questions. They were simply stated along with the answers. It was obvious there was only one correct choice.
